City Care Medical Respite Facility Receives NMTC Funding

OKLAHOMA CITY — Individuals experiencing homelessness in Oklahoma are now closer to having a soft place to land after being discharged from the hospital. Oklahoma City-based MetaFund recently allocated $11 million in New Markets Tax Credits (NMTC) to the nonprofit, City Care, to help finance a 40-bed, integrated-care medical respite facility.

 

City Care has been serving Oklahomans experiencing homelessness since 1996. With the addition of the NMTC allocation, the organization is close to completing a capital campaign to build the project.

 

“Medical respite care addresses an important gap in our community by creating a safe and dignified place for neighbors to continue healing,” said Rachel Freeman, President and Chief Executive Officer at City Care. “This new facility will provide the space and opportunity needed to recover while connecting people to the resources that can help them move forward. We want to ensure that hospital discharge does not result in a return to the streets, but it is a progressive step toward wellbeing and stable housing.”

 

The facility will provide a place where neighbors can recover, connect with housing navigation services and begin moving toward long-term stability. It will operate next to City Care’s existing low-barrier night shelter near downtown Oklahoma City and is projected to open in 2028. 

 

Hospitals absorb significant costs when they are unable to discharge individuals experiencing homelessness. Medical respite programs result in thousands of dollars saved in reduced hospital stays and improved health and social outcomes. City Care is implementing this concept in partnership with local hospital systems and managed care organizations. Mercy, SSM Health, and Humana Healthy Horizons Oklahoma have all committed funding to the project.

 

City Care has also raised $9.8 million in philanthropic and grant support, including a $2 million challenge grant from the Mabee Foundation. BancFirst is providing bridge financing and will also serve as the tax credit investor.

City Care expects to serve at least 300 patients annually at the respite center while operating at approximately 85-90 percent occupancy with stays anticipated to last 30 days. In addition to room and board, services will include behavioral health, chronic-condition and medication management, care coordination, daily wellness checks, on-site clinical services, documentation support, housing navigation, and more.
